Complete the enrollment form to apply for the Proactive Health Access Membership.

For $39/month per household, members receive access to virtual primary care, 24/7 virtual urgent care, virtual behavioral health support, and prescription benefits.

Your membership will begin on the 1st day of the month following successful enrollment and payment. Applications submitted after the 25th of the month may be held for the next available membership start date.

Important: This membership is not health insurance and does not replace major medical coverage.

I agree to the Proactive Health Access Membership Terms & Conditions. I authorize Bullock Services TPA to charge my payment method $39 per month for my household membership.

I understand my membership will begin on the 1st day of the month after my application is complete and my first payment is successfully processed. Applications or payments submitted after the 25th may be held for the next available membership start date.

I understand the Proactive Health Access Membership is not health insurance, does not replace major medical coverage, and should not be used for medical emergencies. For emergencies, I should call 911 or go to the nearest emergency room.
